Employer File an Appeal

To file an online appeal you must know the claimant's social security number and the Appeal ID, which is located in the top right portion of the Department's initial decision letter. If you need help locating this number, call the Appeals Unit at 801-526-9300.

All appeals and hearing information will be sent to the Employer's address of record with the Department. If you receive electronic correspondence, the information will be sent electronically. If you have any questions, call the Appeals Unit.
